There are (annoying) cases in which you get the warning even if
the tab or window is not visible:

  - Open the replace dialog and enter something (the check to 
    determine the state of the "Multiple Files..." button triggers
    the modification check for all files unintentionally).

  - Select something in a tab, hide the tab, modify the file 
    externally and then select something in another tab.

These "bugs" have existed at least since 5.3, but there may be 
other cases. 
IIRC, it's not trivial to fix this, since the checks can be called 
via so many paths that it is hard to decide whether or not a warning
is appropriate, but it can be done.
